Rasmussen, MD, PhD, DMSc; Jacob\u00a0Steinmetz, MD, PhD; for the TRAUMOX2 Trial Group<\/p>\n<p style=\"text-align: center;\">JAMA. 2025 Feb 11;333(6):479-489. doi: 10.1001\/jama.2024.25786.<\/p>\n<p style=\"text-align: center;\"><strong><b>Abstract<\/b><\/strong><\/p>\n<p><strong><b>IMPORTANCE<\/b><\/strong>\uff1aEarly administration of supplemental oxygen for all severely injured trauma\u00a0patients is recommended, but liberal oxygen treatment has been associated with increased\u00a0risk of death and respiratory complications.<br \/>\n<strong><b>OBJECTIVE<\/b><\/strong><strong><b>\uff1a<\/b><\/strong>To determine whether an early 8-hour restrictive oxygen strategy compared with\u00a0a liberal oxygen strategy in adult trauma patients would reduce death and\/or major\u00a0respiratory complications.<br \/>\n<strong><b>DESIGN, SETTING, AND PARTICIPANTS<\/b><\/strong><strong><b>:<\/b><\/strong>\u00a0This randomized controlled trial enrolled adult trauma\u00a0patients transferred directly to hospitals, triggering a full trauma team activation with an\u00a0anticipated hospital stay of a minimum of 24 hours from December 7, 2021, to September 12,\u00a02023. This multicenter trial was conducted at 15 prehospital bases and 5 major trauma\u00a0centers in Denmark, the Netherlands, and Switzerland. The 30-day follow-up period ended\u00a0on October 12, 2023. The primary outcome was assessed by medical specialists in anesthesia\u00a0and intensive care medicine blinded to the randomization.<br \/>\n<strong><b>INTERVENTIONS<\/b><\/strong><strong><b>:<\/b><\/strong>\u00a0In the prehospital setting or on trauma center admission, patients were\u00a0randomly assigned 1:1 to a restrictive oxygen strategy (arterial oxygen saturation target of\u00a094%) (n = 733) or liberal oxygen strategy (12-15 L of oxygen per minute or fraction of inspired\u00a0oxygen of 0.6-1.0) (n = 724) for 8 hours.<br \/>\n<strong><b>MAIN OUTCOMES AND MEASURES<\/b><\/strong>: The primary outcome was a composite of death and\/or\u00a0major respiratory complications within 30 days. The 2 key secondary outcomes, death and\u00a0major respiratory complications within 30 days, were assessed individually.<br \/>\n<strong><b>RESULTS<\/b><\/strong><strong><b>:<\/b><\/strong><strong><b>\u00a0<\/b><\/strong>Among 1979 randomized patients, 1508 completed the trial (median [IQR] age, 50\u00a0[31-65] years; 73% male; and median Injury Severity Score was 14 [9-22]). Death and\/or major\u00a0respiratory complications within 30 days occurred in 118 of 733 patients (16.1%) in the\u00a0restrictive oxygen group and 121 of 724 patients (16.7%) in the liberal oxygen group\u00a0(odds ratio, 1.01 [95% CI, 0.75 to 1.37]; <em><i>P <\/i><\/em>= .94; absolute difference, 0.56 percentage points\u00a0[95% CI, -2.70 to 3.82]). No significant differences were found between groups for each\u00a0component of the composite outcome. Adverse and serious adverse events were similar\u00a0across groups, with the exception of atelectasis, which was less common in the restrictive\u00a0oxygen group compared with the liberal oxygen group (27.6% vs 34.7%, respectively).<br \/>\n<strong><b>CONCLUSIONS AND RELEVANCE<\/b><\/strong><strong><b>:<\/b><\/strong>\u00a0In adult trauma patients, an early restrictive oxygen strategy\u00a0compared with a liberal oxygen strategy initiated in the prehospital setting or on trauma\u00a0center admission for 8 hours did not significantly reduce death and\/or major respiratory\u00a0complications NCT05146700<\/p>\n<p><strong><b>\u7814\u7a76\u80cc\u666f<\/b><\/strong>\uff1a\u5bf9\u4e8e\u6240\u6709\u91cd\u5ea6\u521b\u4f24\u60a3\u8005\uff0c\u65e9\u671f\u8865\u5145\u6c27\u6c14\u88ab\u63a8\u8350\uff0c\u4f46\u8fc7\u5ea6\u4f7f\u7528\u6c27\u6c14\u6cbb\u7597\u4e0e\u6b7b\u4ea1\u98ce\u9669\u548c\u547c\u5438\u5e76\u53d1\u75c7\u7684\u589e\u52a0\u76f8\u5173\u3002<\/p>\n<p><strong><b>\u7814\u7a76\u76ee\u7684<\/b><\/strong>\uff1a\u786e\u5b9a\u4e0e\u5f00\u653e\u6027\u6c27\u7597\u76f8\u6bd4\uff0c\u65e9\u671f\u4fdd\u5b88\u6027\u6c27\u7597\u7b56\u7565\u662f\u5426\u80fd\u51cf\u5c11\u6210\u4eba\u521b\u4f24\u60a3\u8005\u7684\u6b7b\u4ea1\u548c\/\u6216\u91cd\u5927\u547c\u5438\u5e76\u53d1\u75c7\u3002<\/p>\n<p><strong><b>\u7814\u7a76\u8bbe\u8ba1\u3001\u5730\u70b9\u4e0e\u53c2\u4e0e\u8005<\/b><\/strong>\uff1a\u672c\u968f\u673a\u5bf9\u7167\u8bd5\u9a8c\u62db\u52df\u4e86\u76f4\u63a5\u8f6c\u8fd0\u81f3\u533b\u9662\u7684\u6210\u4eba\u521b\u4f24\u60a3\u8005\uff0c\u542f\u52a8\u4e86\u5168\u521b\u4f24\u56e2\u961f\u6fc0\u6d3b\uff0c\u9884\u8ba1\u4f4f\u9662\u65f6\u95f4\u4e0d\u5c11\u4e8e24\u5c0f\u65f6\uff0c\u7814\u7a76\u65f6\u95f4\u4e3a2021\u5e7412\u67087\u65e5\u81f32023\u5e749\u670812\u65e5\u3002\u8be5\u591a\u4e2d\u5fc3\u8bd5\u9a8c\u5728\u4e39\u9ea6\u3001\u8377\u5170\u548c\u745e\u58eb\u768415\u4e2a\u9662\u524d\u57fa\u5730\u548c5\u4e2a\u4e3b\u8981\u521b\u4f24\u4e2d\u5fc3\u8fdb\u884c\u300230\u5929\u7684\u968f\u8bbf\u671f\u4e8e2023\u5e7410\u670812\u65e5\u7ed3\u675f\u3002\u4e3b\u8981\u7ed3\u5c40\u7531\u9ebb\u9189\u548c\u91cd\u75c7\u533b\u5b66\u9886\u57df\u7684\u533b\u5b66\u4e13\u5bb6\u8bc4\u4f30\uff0c\u4e14\u4ed6\u4eec\u5bf9\u968f\u673a\u5206\u7ec4\u60c5\u51b5\u4fdd\u6301\u76f2\u6cd5\u3002<\/p>\n<p><strong><b>\u5e72\u9884\u63aa\u65bd<\/b><\/strong>\uff1a\u5728\u9662\u524d\u8bbe\u7f6e\u6216\u521b\u4f24\u4e2d\u5fc3\u5165\u9662\u65f6\uff0c\u60a3\u8005\u63091:1\u7684\u6bd4\u4f8b\u968f\u673a\u5206\u914d\u5230\u4fdd\u5b88\u6027\u6c27\u7597\u7ec4\uff08\u52a8\u8109\u8840\u6c27\u9971\u548c\u5ea6\u76ee\u680794%\uff09\uff08n vs 34.7%\uff09\u3002<\/p>\n<p><strong><b>\u7ed3\u8bba\u4e0e\u610f\u4e49<\/b><\/strong>\uff1a\u5bf9\u4e8e\u6210\u4eba\u521b\u4f24\u60a3\u8005\uff0c\u4e0e\u5f00\u653e\u6027\u6c27\u7597\u7b56\u7565\u76f8\u6bd4\uff0c\u65e9\u671f\u4fdd\u5b88\u6027\u6c27\u7597\u7b56\u7565\u5728\u9662\u524d\u8bbe\u7f6e\u6216\u521b\u4f24\u4e2d\u5fc3\u5165\u9662\u540e\u5b9e\u65bd8\u5c0f\u65f6\uff0c\u5e76\u672a\u663e\u8457\u51cf\u5c1130\u5929\u5185\u6b7b\u4ea1\u548c\/\u6216\u91cd\u5927\u547c\u5438\u5e76\u53d1\u75c7\u3002<\/p>\n<p>(\u5b59\u9a8e
